In inferior alveolar nerve (IAN) blocks, the needle traverses 20 to 25 mm of soft tissue. Thin 30G needles can deflect several millimeters away from the target sulcus, causing incomplete anesthesia. The larger core of a 27G long needle resists deflection, ensuring accurate deposition near the mandibular foramen.
§ 2. Positive Aspiration and Vascular Safety
Accidental intravascular injection of local anesthetic with epinephrine can cause systemic toxicity and tachycardia. The larger lumen of a 27G needle provides lower resistance, allowing rapid blood flashback during aspiration tests before anesthetic deposition.
